In accordance with he Florida Statutes Chapter 723.076(2). we are writing to request that you provide our homeowners' association with the following information in writing by certified mail, return receipt requested.

 
 
1. The name of the park owner
2. The address of the park owner
3. The name and address of the park owner's agent for service of process
4. The legal description of the mobile home park
